JBCCS is a one stop assessment service to assist our clients in preparing a care plan that addresses individual support needs. We also provide assistance in putting participants in touch with the services they require, for example the care and assistance with daily tasks offered by JBCCS, as well as liaising on behalf of participants with community and Government resources available to them.
JB Community Care Services provides in home care services with the aim to ensure participant using JBCCS support are provided person-centered services, allowing people to tell their story once rather than taking a program-focused approach.
JB Community care services provide high quality care services depending on participant’s individual support plan assessments; which includes but not limited to supporting participant with their daily living tasks.
These support may include assistance with cooking, cleaning and shopping and/or assistance and support you need with meeting your loved ones and catching up with your friends by providing Community Access as per every individual needs. JBCCS as an organisation is committed to increase client participation within the community and encourage them to live a joyful environment and keeping them active to improve their daily life.
